Initial Consultation Forms

Yes there will be minimal paper work but by filling these forms out now will save time in the consultation.

Yes there is some paperwork, but it is only a couple of pages.

To avoid delay when coming in for your first appointment, simply download the consultation form before your first visit.  After you have filled the form in please do not forget to hit “File” and then “Save As” before emailing the completed forms back to me before your first consultation.

Otherwise print out the forms and fill them in manually and either fax the forms through to me on (07) 3878 9928 or just bring the completed forms with you to your initial appointment.
